Some components disappeared when adding items.

When I tried to add a choice component by adding items, the choice component was not in the list.
Where did the Choice component for adding items go?
By the way, when editing items, I can add a Choice component.

Choice Component

My app’s URL: It seems the choice component has changed. It doesn’t display the header from the spreadsheet anymore. This is mainly ideal for when asking multiple questions requiring a dropdown menu. The choice component only displays the answer and not the question- how would the person know the question they are providing the answer for?
