Form creating two rows of data

Hi,

When I submit this form on this screen, it creates two rows of data for some reason. And only one of those rows has the current Date & Time filled out while the other one is empty.

Any idea why this is happening?

Also this form is located on the detail page of “Company A”. Is there a way to auto-populate the “company” field in the form with “Company A” or the company detail page that I am currently on?

Remove the extra Add Row action. Forms already add rows on there own.

Also add the necessary special value components to get the date and time.

Same for the company. Add the appropriate column value component to the form.
