If you tried to change the layout from a grid layout to a table in a collection component then your add form will get reset
So am checking first if this is already reported before I submit ticket as per @SantiagoPerez to check the community as a reference

I am unable to reproduce this.
If you are seeing this, please submit a ticket with a video on how to reproduce it.
Engineering is working on a fix for this issue.
