Default value In a form

Trying to add a default value in a form to know From which client the person fill the form , So everyone have the same Form and have to share it to there Clients.
So Here we have 2 Tables (User and Clients )
User have unique Id . I want to Know If it is possible when the User share his Form add a Slug (unique Id of the User) . And To add it at the default value in the form .
