Buenas tardes, se esta tratando de crear un sondeo donde la persona pueda ingresar toda la informacion requerida como “la imagen previa adjuntada como referencia”, solo que reemplazando el texto de pregunta por imagen y manteniendo la parte del relleno de formulario. Si hay algun modo de replicarlo o algo parecido.
