I did get reviews via form.
See attachments. Note the number of rows (78 vs 73 after)

I am planning to redo the reviews sheet. May be I made a mistake somewhere because the template is also not showing user sheet columns like Jeff suggested.

@ThinhDinh: The formula you used with ARRAYFORMULA and VLOOKUP works.
Glide was adding new review data after blank rows (around 900). I guess this was creating issues during sync and it was deleting its own content. I deleted blank rows in google sheet and now it works just fine. Thanks for your help.

Make sure you delete all empty rows from your sheets :smile:
