Is it possible? Different Form Questions for each Collection Item

I’ve got a page with a collection item on it. I have it set so you can click on each collection item to fill out a form. Is there a way to show different forms for each collection item? I changed a part of the form on one of the items and it seemed to show up that way for all of the forms for that collection on that page then.

Any help would be greatly appreciated!

You will need to create a custom action with several IF branches to open different forms based on the collection item chosen.
