How do I create a form that will fill in rows on the glide tables

I want to be able to create a form that has 6 different columns each associated with a different question which will be on the forms. As you submit each form I want it to fill out the associated row for that form. As each form is completed the rows continue with new information each associated with that form. Also, from there I want to have a tab that will show all the information presented in each row and column. How would I go about doing this. I will attach a picture of what the glide table looks like. Also, how do I make the form appear to be the only thing on the tab with no need to click any button to access the form.

There’s no way to open a form environment right when you click the tab. You may have to build a custom form for that.
