Form button issue

Hi guys !
I hope someone can help me. I’m trying to create an app for my students (with a Youtube tutorial) and I want to add a form button but it doesn’t exist anymore or at least I can’t find it.My goal here is to give them points (reason, day, numbers of points) or remove points.
All I can see is “button block” and "button…

You can just add a button and then configure the action to open a form screen, if using a form is your intention.
