Create Purchase Order PDF

Hi,

I want to allow a user to create a purchase order.

The user has to be able to add the items, quantity, unit price, etc as seen on the image. I am having a tough time coming up with the databases and linking. Would appreciate if someone who has worked on something similar can provide some help.
