Hello, I am making a treatment plan app for people who can create their own medical treatment plan for themselves, by completing assessment tests that can determine what treatment would work best. Any suggestions on how to get the assessments started? I have the questions, but I am making sure that once they finish, depending on their score on the assessment would determine what plan would suite best for them.

It sounds like you just need a normal form for people to do the assessment, then convert them to a numerical score. Then you determine what you do on the logic side with that score (to show them the right plan).
