Basic Form Report App

Hi everyone
I am looking for a glide app that will just allow other members of a club to report things. So a form with 10 fields say that can take data in / store in the excel sheet then clear on the apps front end ready for another submission. So just like a google form really but using the glide app to facilitate users submitting via their mobile devices whilst out on site. Any ideas or pointers would be appreciated.
