# Same form, different locations?

I’m not sure exactly how to describe this, but if I want users to fill out the same form, but from different locations (like a button on the home screen, and/or menu item), it appears I need to create multiple instances of the same form button. Is that correct? I can’t figure out how to duplicate form in different places.
